Information for Patients

Patients are the NUMBER ONE priority at Thousand Oaks Radiology.   Here you will find helpful information to help you prepare for your visit.

Prepare for your visit.

  • Speak to your doctor.  Your doctor has ordered you an exam for a reason.  The more you know and understand about your condition, the better you'll be able adequately prepare yourself. 
  • Confirm your appointment.  Our friendly staff will give you a reminder call the day prior to your exam.   Please let us know any changes to your schedule - we have other patients who would be willing to take your appointment time should you have a change in your plans.  We will assume that you will make your appointment, unless you tell us otherwise.
  • Check your insurance.  Some insurance carriers require "Authorization" prior to any exam.  We are happy to assist in this process - please contact our offices or contact your doctors' office for help.   If you choose to use our assistance, we will need the following, faxed to:  (805) 482-2431

    1. Copy of your insurance card.  Both the front and back of the card.
    2. A copy of the doctors' order, Rx, or exam codes.
    3. Your doctors' name.

Patient Forms

The following patient forms, are needed for your registration upon arrival.  If you want to fill them out ahead of time, then please download and print the appropriate forms listed below.

  • All patients should have a copy of the General Registration Information Form.  Returning patients:  If you have filled this out in the past, then we may need only changed demographic information since your last visit.  Please check with our offices to confirm that we have your correct information.
  • All MRI / CT patients will need to fill out a Medical History Questionnaire.  Our radiologists use this information in their reporting.
  • We will deliver your results and any necessary images to your doctor's office.  Should you wish to transport them yourself, we will need you to sign a Film Release Form, so we can track their location(s).
  • If you are visiting for Mammography, then we will need a copy of the Mammography Screening Form.  
  • As a courtesy to our patients, we can bill your insurance carrier.   The Insurance Release Form, authorizes the release of necessary medical information to bill your plan directly.
